Cera is Europe’s largest provider of digital-first domiciliary care, delivering 50,000 visits to people in their homes each day. Every time carers visit someone, they fill in a report answering questions about their wellbeing using the Cera app. We worked with Cera to investigate how this data could help prevent avoidable hospitalisations amongst its users.

Solution

Cera’s team needed a reliable method of predicting which of its users might need a proactive intervention. Our solution produces a prioritised list of patients based on their hospitalisation risk. Cera’s nurses can now use this list to arrange low-cost, preventative interventions - such as GP telephone consultations, district nurse visits, or pharmacist medication reviews - to reduce the need for future hospital admissions.

Impact

The solution can predict 80% of admissions up to a week in advance, helping to cut hospitalisations in half with simple and low cost interventions. Compared to clinicians alone, the solution proved to be 2.6x more accurate in its assessments, reducing the strain on the NHS. These impacts were independently reviewed and published in a leading medical journal, showing the real world outcomes of leveraging AI safely. This work has also been highlighted in the The Times. The article showcases the profound impact it has had on individual users, and the NHS as a whole.
